A WOMAN with learning difficulties was physically abused at a council-run day centre by two members of staff, an Ombudsman's report has revealed. Carmarthenshire County Council was guilty of catastrophic failures in its handling of the case and its treatment of the whistleblower, referred to in the report only as "Ms West", according to Peter Tyndall, the Public Services Ombudsman for Wales.

In a damning 105-page report, Mr Tyndall describes a culture in which incidents of abuse were tolerated.
