Summary
A former farmer hanged himself from his bed frame in a secure hospital despite being on a 15-minute watch. Thomas Charles, of Mathry, was found kneeling by the side of his bed at Bro Cerwyn Hospital in Haverfordwest by a staff member who had brought his supper. Attempts by staff at the Pembrokeshire and Derwen NHS Trust- run hospital and by ambulance paramedics to revive the 70-year-old failed.
