A HARD-HITTING investigation into the state of school toilets slipped through the net in the wake of the Clywch inquiry and the death of a child in a school bus accident.

The E.coli public inquiry yesterday heard the Welsh Assembly Government chose to ignore the recommendations of a 2004 Lifting the Lid report by Peter Clarke, the Children's Commissioner for Wales, into the appalling state of toilets in Wales' 1,800 schools.
